The Office of the City Civil Registrar is mandated to carry out the civil registration program of the city; develop plans and strategies particularly those which have to do with civil registry programs and projects; accepts all registrable documents and judicial decrees/legal documents affecting the civil status of persons; transmits to the Office of the Civil Registrar-General duplicate copies of registered documents; receives applications for the issuance of marriage license; and conduct educational campaigns for vital registration and assists in the demographic and other statistics information of the city.

BRIEF HISTORY
HISTORICAL BACKGROUND
The office of the City Civil Registrar in Zamboanga City was created under Ordinance No. 188, series of 1984. Prior to this, the Secretary to the Mayor served as Ex-Officio Civil Registrar.
The Local Government Code of the Philippines of 1991 (RA7160) strengthened the existence of the office and provides that in highly urbanized cities, like Zamboanga City, civil registration shall be performed by the City Civil Registrar, whose office is listed among the mandatory offices of the local government unit.
The registration of civil registry documents of births, marriages, deaths and vital events affecting the civil status of people in the local level is being done by the City Civil Registrar.

Petition for Correction of Clerical Error and Change of First Name / Petition to correct the Day and/or Month in the Date of Birth or Sex under the provisions of Republic Act No. 9048 & 10172
FILING OF PETITION FOR CORRECTION OF CLERICAL ERROR AND CHANGE OF FIRST NAME/ PETITION TO CORRECT THE DAY AND/OR MONTH IN THE DATE OF BIRTH OR SEX UNDER THE PROVISIONS OF REPUBLIC ACT NO. 9048 & 10172.
ABOUT THE SERVICE
R.A. 9048 amends Articles 376 and 412 of the Civil Code of the Philippines. Article 376 prohibits the changing of name or surname of a person without judicial authority, while Article 412 prohibits any correction or change of entry in a register without a judicial order. The amendatory provision of R.A. 9048 is Section 1, which provides:
Section 1. Authority to correct clerical or typographical error and change of first name or nickname – No entry in a civil register shall be changed or corrected without a judicial order, except for clerical or typographical errors and change of first name or nickname which can be corrected or changed by the concerned city or municipal civil registrar or consul general in accordance with the provisions of this Act and its implementing rules and regulations.
R.A. 10172 authorizes the correction of errors in sex or day and month of birth in the birth certificate.

- Baptismal Certificate
- Marriage Contract
- Birth Certificates of children, if married
- School Records
- Voters Record
- IDs
- Other documents showing correct first name
- NBI & Police Clearance for change of name purpose
- If employed, Certificate of Employment with no derogatory records with the employer
- If, Self-employed, Affidavit of Self-employment
CCE – R.A.10172, requirements for correction of sex,date and month of birth in the birth certificate (present original copies and provide 2 photocopies of each):
- PSA and LCRO copy of Birth Certificate
- Earliest school record (Elem. & HS Form 137)
- Medical Records (Immunization/Laboratory Exams/Previous Hospitalization)
- Baptismal Certificate/Paggunting (Muslim Rites)
- Clearances from the following:
- Employer – Certification of No Pending Cases (If not employed, Affidavit of Non/If employed, Affidavit of Self-Employment)
- National Bureau of Investigation
- Philippine National Police
- Medical Certification from an accredited gov’t physician certifying that the petitioner has not
- undergone sex change or sex transplant
- Other documentary evidences which may show the correct sex or date and month of birth such as:
- Marriage Certificate
- Birth Certificate of children
- Valid IDs
